- Author of the work you request permission to quote or reprint
- Title of the work you wish to quote or reprint

If you wish to reproduce in another publication material copyrighted by Penn Press, you may complete our Permission Request Form or else you may print out our Reprint Permission Form, available in PDF format. After completing the printed PDF form, you may mail or fax it to the attention of:
